AuditTrailr
AuditTrailr is a web app (with optional EHR-embedded launch) that continuously checks clinical documentation for compliance and audit readiness before claims go out. It focuses on the unglamorous but expensive gap: missing elements, internal inconsistencies, cloned text, weak medical necessity language, time-based coding pitfalls, and signature/authentication issues. The app ingests notes via HL7/FHIR or flat-file exports, runs rule packs by specialty (e.g., E/M, PT/OT, behavioral health), and produces a defensible “audit packet” with evidence links to the chart. It also tracks who edited what and when, creating a clean chain-of-custody that compliance teams can trust. This is an AI + traditional rules product: deterministic checks for known requirements, plus AI to flag ambiguous risk areas and suggest safer phrasing without rewriting the clinician’s voice.
