BenefitBridge is a mobile + web app that helps frontline health equity teams (community health workers, Medicaid navigators, safety-net clinics) quickly determine what a patient is likely eligible for and what to do next—without pretending to replace official determinations. Users enter a few verified data points (household size, income range, address, pregnancy/disability flags, language, immigration-sensitive handling), and the app generates a plain-language checklist: programs to consider (Medicaid/CHIP, SNAP/WIC, local charity care, transportation, utility assistance), required documents, and the fastest submission path by state/county. It also tracks follow-ups, sends SMS reminders in the patient’s language, and produces simple reports on where drop-offs happen (missing documents, long call waits, denied for paperwork). It’s realistic: the value is workflow speed and fewer failed applications, not “magical approvals.”
