CareBridge is a mobile + web app that helps families and care teams support someone with psychosis without turning into amateur clinicians. The core is a structured, low-friction daily check-in the patient can complete in under 60 seconds (sleep, stress, medication taken, “voices/paranoia intensity” slider, and one free-text note). The app converts this into a simple “support status” for caregivers, with clear, pre-agreed action steps (e.g., “do nothing,” “offer a ride,” “call case manager,” “use crisis plan”). It also stores a one-page crisis plan, preferred de-escalation phrases, triggers, and emergency contacts, accessible offline. An optional AI layer summarizes trends and flags abrupt changes, but it never diagnoses or gives medical advice. The realistic value is coordination and earlier intervention, not predicting relapse with magic accuracy.
