CareChord

CareChord is a mobile + web app (traditional app with light AI) that turns messy family caregiving into a single source of truth. It combines a shared care plan, a shift/task board, and a simple daily log so siblings stop arguing over “who did what” and clinicians get clearer updates. The core is a timeline-style journal: meds given, meals, mood, vitals, incidents, appointments, and notes—each entry can be posted by any approved caregiver with photo attachments. Light AI helps by summarizing the last 7 days into a concise “doctor-ready” brief and flagging patterns (missed meds, repeated falls, sleep decline) without pretending to diagnose. It also includes permissioned access (family vs. paid helpers) and an export pack (PDF/CSV) for appointments or insurance paperwork. This is not a replacement for medical systems; it’s the coordination layer families actually use daily.
