It ingests appointment history, basic vitals, diagnoses, labs, and patient messages from common EHR integrations, then generates a prioritized outreach list: overdue screenings, uncontrolled chronic conditions, missed follow-ups, and medication non-adherence signals. Staff get simple scripts, recommended timing, and one-click tasks (call/text/portal message) with audit trails. Patients receive plain-language reminders and a lightweight check-in flow to confirm barriers (cost, transport, side effects) so the clinic can route to the right help. This is not a diagnostic tool; it’s an operational layer to reduce leakage and improve quality metrics. The MVP focuses on one specialty (family medicine) and a narrow set of high-value gaps to avoid overpromising.
