The next caregiver gets a concise “what changed since last time” summary and can confirm understanding before starting tasks. Built-in reminders catch common misses (missed PRN meds, hydration, fall-risk steps) and a simple timeline makes it easy to answer family questions without digging through texts. An AI layer can turn messy notes/voice dictation into standardized entries and flag inconsistencies (e.g., “ate well” vs. “no appetite” same day). This is not a full EHR; it’s a lightweight handoff tool that fits real home care workflows and reduces preventable errors.
