CareLedger is a mobile + web app for home care agencies and independent caregivers to capture defensible, audit-ready visit documentation with minimal friction. Caregivers clock in/out with geofenced verification, then complete a structured “care note” that’s fast: tasks done, vitals (optional), incidents, meds assistance (non-clinical), and client mood/ADL status. Family members (optional) get a read-only timeline and alerts for missed visits or notable changes. The AI layer turns short voice notes into clean, standardized documentation and flags inconsistencies (e.g., time-on-task vs. visit length) before submission. It also generates weekly summaries for agency supervisors to review quickly. This is not a full EHR and won’t pretend to be; it focuses on the messy middle: proving services happened, reducing rejected claims, and making supervision easier. Expect heavy compliance needs (HIPAA) and slow sales cycles, but the value is concrete if it reduces admin time and payment disputes.
