CareNudge is a web + mobile care coordination app for small home health agencies and care managers who juggle too many patients across too many systems. It turns a patient’s plan of care into a simple, trackable checklist of tasks (meds, follow-ups, labs, equipment, transportation) with owners, due dates, and escalation rules. The app focuses on “last mile” coordination: confirming appointments happened, verifying prescriptions were picked up, and documenting barriers (cost, no ride, confusion). It supports secure messaging with caregivers and patients, but keeps it structured so conversations become tasks, not endless threads. Integrations are realistic: start with calendar + SMS/email, then add EHR connectivity only where it’s feasible. The product is intentionally lightweight—built to replace spreadsheets and sticky notes, not to be a full EHR. An AI layer can summarize updates and suggest next actions, but humans stay in control.
