CarePing is a web + mobile care-coordination app that turns post-discharge and chronic-care plans into a simple, trackable checklist shared across the care team. It focuses on the messy middle: confirming appointments were actually scheduled, meds were actually picked up, transportation is arranged, home services started, and red-flags are escalated fast. Patients and caregivers get SMS/app reminders and a “one-tap” confirmation flow; coordinators see a live queue of unresolved tasks with deadlines and risk flags. The app ingests discharge instructions and referral notes, then suggests tasks, owners, and follow-up timing. It also provides lightweight documentation (who contacted whom, when, outcome) to reduce phone-tag and audit pain. This is not a full EHR replacement—it's a narrow layer that plugs into existing systems and makes follow-through visible and measurable.
