CareProof

CareProof is a web + mobile app for clinics, community orgs, and public health teams to document and quantify “time-to-care” inequities (referral to appointment, appointment to treatment, follow-up completion) by neighborhood, language, insurance type, and other locally relevant factors. Staff log or import referral events, appointment outcomes, and barrier reasons (no transport, childcare, work conflict, denial, language mismatch). The app generates simple, defensible reports for grant reporting and internal improvement: where delays happen, which populations are disproportionately affected, and what interventions move the needle. An AI assistant helps normalize messy notes into standardized barrier categories and flags data quality issues, but the core value is structured workflow and audit-ready metrics. This is not a consumer app; it’s a practical operations and reporting tool that turns anecdotes into measurable equity gaps and tracks whether fixes actually reduce delays.
