CareShift

CareShift is a mobile + web app for hospice and palliative teams to coordinate real-world care tasks across clinicians, aides, and family caregivers. It replaces scattered phone calls, sticky notes, and group texts with a single shared plan: visit schedules, medication pickup, symptom check-ins, equipment delivery, and who is responsible for what. The app includes a lightweight “patient status board” with today’s priorities, recent changes, and escalation rules (e.g., when to call the on-call nurse vs. 911). It also generates a simple daily summary for families who feel overwhelmed and forget details. This is not a miracle cure: adoption is hard in hospice, and workflows vary. The MVP focuses on one job—reducing missed handoffs and duplicate work—without trying to be a full EHR or billing system.
