CareShift is a mobile + web app that helps families manage the messy life-stage transition of an aging parent: from “mostly independent” to needing in-home help, assisted living, or memory care. It centralizes the practical stuff people scramble for during a crisis—medication list, diagnoses, insurance cards, legal docs, emergency contacts, care preferences, and a shared task board for siblings. The app includes guided checklists for common transition moments (post-hospital discharge, new diagnosis, fall risk, driving retirement) and a “what to do next” planner that turns a situation into a prioritized action list. It also tracks appointments, symptoms, and incidents to create a clean timeline you can share with clinicians. This is not therapy or vague wellness content; it’s operational coordination for families who are overwhelmed and time-poor.
