ChartDiff

ChartDiff is a web app (with optional desktop agent) that detects and reconciles documentation differences when clinicians must chart in multiple systems (hospital EHR, specialty EHR, rehab, home health, billing portals). It ingests CCD/FHIR data plus exported note text, then highlights what changed, what’s missing, and what conflicts (med lists, allergies, problem lists, orders, diagnoses). The MVP focuses on “diff + reconcile” workflows: pick the source of truth, push updates back where possible, and generate an audit trail when write-back isn’t allowed. It’s an AI+traditional app: AI suggests mappings (e.g., diagnosis wording, med equivalence) and flags likely copy-forward errors, but users must confirm changes. This is not a full EHR replacement; it’s a pragmatic layer for organizations stuck with fragmented systems and constant mergers, referrals, and care transitions.
