ComfortChart is a web + mobile app for hospice and palliative teams to coordinate symptom management and family communication without relying on scattered texts, paper binders, or EHR workarounds. It provides a simple shared care plan: current meds and PRNs, symptom scores (pain, dyspnea, agitation, nausea), what to do “if X happens,” and who to call. Families can log symptoms in plain language, get step-by-step guidance approved by the hospice, and request a nurse visit with context attached. Clinicians get a triage dashboard that flags risk patterns (e.g., escalating PRN use, repeated dyspnea entries) and generates concise shift handoff notes. This is not trying to replace the EHR; it’s a lightweight coordination layer that actually gets used at 2 a.m. when the caregiver is panicking and the on-call nurse needs clarity fast.
