ConsentLedger is a web app with a lightweight mobile companion that standardizes, captures, and audits patient consent documentation across clinics and hospitals. It provides configurable consent templates (procedure-specific, state-specific, and payer-specific), guided workflows for staff, and a tamper-evident timeline of who collected what, when, and how. Patients can review and sign on a tablet/phone, attach photos of IDs when required, and receive a copy automatically. For compliance teams, the app surfaces missing elements (witness, interpreter, risks/benefits, alternatives, capacity) and generates an exportable “consent packet” for audits, claims disputes, and legal requests. This is a combination traditional + AI app: AI is used only to detect missing fields, inconsistencies, and risky wording—not to invent clinical content. It integrates with EHRs via FHIR where possible, but can run standalone for smaller practices.
