DecisionTrail

DecisionTrail is a web app (with optional Slack/Teams integration) that captures and organizes the “why” behind decisions: what was decided, by whom, when, based on which inputs, and what assumptions/risks were accepted. It turns messy meeting notes, scattered docs, and chat threads into a searchable decision log tied to projects, policies, and metrics. Teams can create lightweight decision records with required fields (owner, approvers, options considered, rationale, expected impact, review date). An AI assistant can draft a decision record from a pasted transcript or thread, but humans must confirm final wording to avoid hallucinations. The app supports decision reviews (did it work?), recurring governance cadences, and audit-ready exports. This is not a knowledge base; it’s a system of record for decisions and their outcomes, reducing re-litigation and organizational amnesia.
