DecisionTrail is a web app (with Slack/Teams integrations) that captures, structures, and tracks organizational decisions so they don’t disappear in threads, decks, or meeting notes. Users log a decision in under 60 seconds: what was decided, why, owner, impacted teams, risks, and review date. The app then creates a searchable “decision record” with a lightweight approval trail, links to source docs, and automatic reminders when assumptions expire. A simple dashboard shows decisions by team, status, and upcoming reviews, plus an audit-friendly export for compliance-heavy orgs. This is not another knowledge base; it’s a focused system of record for decisions and their rationale. AI is used only to summarize meeting notes and suggest decision templates, but the human owner must confirm before anything is stored.
