DoseFence

DoseFence is a lightweight clinical decision support web app that focuses on one high-impact problem: medication dosing safety at the moment of prescribing. It plugs into EHR workflows via SMART on FHIR and runs real-time checks for renal/hepatic adjustment, age/weight-based dosing, duplicate therapy, and high-risk drug-lab mismatches (e.g., potassium and spironolactone). Instead of noisy alerts, it uses a tiered “stoplight” system and requires a brief rationale only for red-level overrides, creating an audit trail for quality and safety teams. The app supports local formulary rules, configurable thresholds, and institution-specific pathways, so it can match how a hospital actually practices. It also provides a weekly safety digest showing top override reasons, common near-misses, and rule candidates to refine—helping reduce alert fatigue over time. This is an AI + traditional rules app: rules for determinism, AI for summarizing context and recommending safer alternatives.
