FailForward is a web app (with optional Slack integration) that helps teams capture failures while they’re still fresh, then converts them into actionable “anti-pattern” playbooks. Users log an incident in 3 minutes: what happened, what was expected, impact, contributing factors, and what they’ll do differently next time. The app uses AI to summarize, extract root-cause themes, and generate a one-page learning brief plus a checklist for prevention. A lightweight “blameless” workflow nudges managers to focus on systems, not people, and tracks whether follow-up actions actually get done. Over time, FailForward builds a searchable library of lessons learned, tagged by product area, process, and risk type, so new hires and cross-functional teams can avoid repeating the same expensive mistakes. It’s not therapy; it’s operational learning with receipts.
