FailStamp
FailStamp is a web app (with optional mobile companion) that prevents process failures by requiring lightweight, time-bound proof for critical steps in high-risk workflows (e.g., reconciliations, approvals, safety checks, release steps). Instead of long SOP documents nobody reads, FailStamp turns key controls into “stamps” that must be completed with evidence: a screenshot, file hash, system log pull, photo, or e-signature. It integrates with common tools (Jira, ServiceNow, Slack/Teams, Google Workspace/Microsoft 365) to auto-capture context and reduce manual updates. When a step is skipped, late, or completed without required evidence, FailStamp escalates to the right owner and creates an audit-ready trail. This is not a magic fix: it won’t repair broken culture or understaffing, but it will make failures visible early and make accountability enforceable without heavy bureaucracy.
