FlarePlan

FlarePlan is a mobile + web AI-assisted chronic disease “flare-up” management app for conditions like COPD, heart failure, IBD, and rheumatoid arthritis. Instead of generic habit tracking, it focuses on short-term deterioration detection and action plans. Users log a 30–60 second daily check-in (symptoms, weight, peak flow, stool frequency, pain, meds taken), optionally pulling vitals from Apple Health/Google Fit. The app flags risky trends, then walks the user through a clinician-approved action plan (e.g., rescue inhaler steps, hydration, diet adjustments, when to call the clinic). It generates a clean weekly summary for caregivers and clinicians, reducing appointment time wasted on “how have you been?” The hard truth: it won’t replace medical care and it won’t prevent all exacerbations, but it can reduce missed warning signs and improve adherence to agreed plans.
