FormFast

FormFast is a web + mobile app (clinic-facing web dashboard, patient mobile/web intake) that converts messy PDFs, scanned packets, and Word docs into clean, mobile-friendly intake forms and consent flows. Clinics upload any form set, and the system auto-detects fields, signatures, checkboxes, and required logic (e.g., show pregnancy questions only when relevant). Patients receive a secure link to complete forms before arrival, with reminders and a “finish later” option. Staff get a real-time completion view and a downloadable, audit-friendly PDF plus structured data export to EHRs via common standards. This is not a magical EHR replacement—it's a narrow wedge: eliminate manual form rebuilding, scanning, and data re-entry. The product lives or dies on accuracy, speed, and easy clinic rollout, not flashy features.
