GapCheck

GapCheck is a web app (with optional mobile-friendly views) for clinics and small health systems to run practical screening programs without buying an expensive enterprise population-health suite. It ingests patient lists from common EHR exports (CSV) and basic claims/eligibility files, then maps them to guideline-based screening rules (e.g., colorectal, cervical, breast, diabetes) and produces an outreach-ready worklist. Staff can segment by risk, last visit, payer, language, and contactability, then send reminders via SMS/email/print vendors and track outcomes (scheduled, completed, declined, unreachable). The app includes an audit trail and “why flagged” explanations to reduce clinical pushback. It’s not magic: data quality and EHR variability are real, so GapCheck focuses on being reliable with messy inputs, providing clear exceptions, and making it easy to iterate monthly. AI is used only to normalize messy codes/text and suggest rule mappings, not to make clinical decisions.
