GapRadar

GapRadar is a web app (with optional desktop-friendly export tooling) that helps small-to-mid healthcare organizations run practical population health analytics without a full enterprise platform. It ingests claims and basic EHR exports (CSV/FHIR where available), then produces a prioritized “care gap” worklist: overdue screenings, chronic condition monitoring lapses, medication adherence risk, and missed follow-ups. The AI layer summarizes why a patient was flagged and suggests next-best actions aligned to common guidelines, but the core logic remains rules-based and auditable to satisfy clinical and compliance scrutiny. It also tracks outreach outcomes (scheduled, completed, refused, unreachable) and shows simple ROI metrics like avoided ED visits proxies and quality measure movement. This is not a replacement for Epic/Cerner population modules; it’s a lightweight, implementation-friendly gap finder and outreach tracker for organizations that can’t afford or can’t successfully deploy the big suites.
