HFDischarge

HFDischarge is a mobile + web app that helps cardiology and hospitalist teams run a reliable 30-day post-discharge program for heart failure patients. It standardizes what usually falls through the cracks: medication reconciliation, guideline-directed therapy titration reminders, daily weight/symptom check-ins, and fast escalation rules to a nurse or clinic when risk rises. Patients get a simple daily flow (weight, dyspnea, edema, BP if available) and clear instructions; clinicians get a dashboard that flags only the patients who need attention, plus templated outreach scripts and documentation notes. This is not a magic “AI diagnoses you” product; it’s operational software with light AI to summarize trends and draft messages. The goal is measurable: fewer avoidable readmissions, fewer missed follow-ups, and better adherence to HF care pathways without adding a full-time coordinator.
