IntakeRelay is a web + mobile telehealth intake and triage app that gets patients “visit-ready” before they ever meet a clinician. Patients receive a secure link to complete adaptive questionnaires, upload photos (rash, wound, meds), verify pharmacy and insurance basics, and run a device check for camera/mic. The system flags missing essentials (e.g., no vitals, unclear photos, incomplete medication list) and routes the patient to the right visit type (async, video, urgent in-person) based on configurable rules. Clinicians get a concise pre-visit brief with red flags, timeline, and structured data that drops into the EHR via FHIR when possible. This is not a magical diagnosis tool; it’s a workflow product that reduces wasted appointments, shortens visit time, and improves documentation quality. It’s realistic because it focuses on operational reliability, not clinical promises.
