LoopLedger

LoopLedger is a web app (with optional Slack/Teams integration) that helps teams capture, analyze, and actually reuse learnings from failures. Instead of long postmortem docs nobody reads, it turns each incident into structured “learning cards” with: what happened, contributing factors, decision points, and concrete prevention experiments. It then tracks whether those experiments were implemented and whether they reduced recurrence. The AI layer summarizes raw inputs (tickets, chat logs, timelines) into a draft narrative and suggests missing questions, but humans approve everything. A lightweight “recurrence radar” flags when a new incident resembles past ones, so teams can reuse mitigations immediately. Reporting focuses on trends (recurring causes, time-to-learn, action completion rate) rather than vanity metrics. This is not a magic fix: it works only if leadership enforces follow-through and teams are willing to be transparent.
