MediLedger is a mobile + web app for maintaining a single, accurate medication list that’s easy to share with clinicians and caregivers. Users can add meds by scanning the pharmacy label, typing, or importing from a pharmacy/health portal when available. The app standardizes entries (drug name, dose, route, frequency, indication), flags duplicates, and highlights common “list rot” issues like expired antibiotics, PRN confusion, and multiple strengths of the same drug. It generates a clinician-friendly PDF and a share link with time-limited access, plus an emergency lock-screen card. This is not a pill reminder; it’s a “source of truth” for medication reconciliation before appointments, hospital admissions, and transitions of care. Data export is straightforward so users aren’t trapped.
