MedListSync

MedListSync is a mobile + web app that helps patients keep a single, verified medication list that stays consistent across doctors, pharmacies, and caregivers. Users scan pill bottles, import pharmacy refill texts/emails, and manually confirm what they actually take (including supplements). The app generates a clean “med reconciliation” summary for appointments and hospital visits, with dosage, schedule, prescriber, and last-filled date. It flags common issues like duplicate therapies, expired meds, and dangerous combinations, but it does not pretend to replace a clinician. The reality: medication lists are often wrong in EHRs, and patients pay the price during transitions of care. MedListSync focuses on accuracy, audit trails, and sharing controls so clinicians can trust it, and caregivers can help without chaos.
