MedRecon

MedRecon is a mobile + web app for medication reconciliation focused on older adults moving between hospital, rehab, home health, and primary care. It lets a clinician or care coordinator build a single “source of truth” med list by importing from EHR/FHIR when available, then confirming with the patient/caregiver using a guided interview and photo-based pill verification. The app flags high-risk issues common in geriatrics—duplicate therapies, anticholinergic burden, renal-dose concerns, and dangerous interactions—then generates a clean, shareable med list and a short action plan for the next provider. It also tracks who confirmed each item and when, reducing blame games during readmissions. This is not a consumer pill reminder; it’s a workflow tool aimed at reducing preventable adverse drug events and readmissions, where the financial incentives already exist.
