MedReconcile
MedReconcile is a web + mobile clinical decision support tool focused on medication reconciliation at admission and discharge—where errors are common and costly. It ingests a patient’s current med list (EHR export or pharmacy history feed), flags duplicates, interactions, renal/hepatic dose issues, and high-risk combinations (e.g., anticoagulants + NSAIDs). It then generates a clinician-ready reconciliation checklist and a patient-friendly “what changed and why” summary to reduce confusion and callbacks. The AI component suggests likely intended regimens by comparing prior fills, problem list, labs (eGFR, LFTs), and typical guideline dosing, but always requires clinician confirmation. The app is designed for busy workflows: quick scanning, one-tap accept/decline, and an auditable rationale log. Realistically, winning depends on tight EHR integration and proving measurable reduction in adverse drug events and readmissions.
