MedRelay
MedRelay is a mobile + web app that helps older adults and their families keep medication lists accurate across doctors, pharmacies, and caregivers. The core problem is that med lists are often wrong: outdated dosages, duplicate drugs, and missing OTC supplements. Users can snap photos of pill bottles and discharge papers, then the app (AI-assisted) extracts names/doses and flags conflicts, duplicates, and “looks off” changes. A shared, permissioned timeline shows who changed what and when, so family members aren’t guessing. It also generates a clean “bring-to-appointments” PDF and a pharmacist-friendly list. This is not a replacement for clinicians; it’s a coordination tool that reduces preventable errors. Realistically, adoption will be highest when a family caregiver is involved, because seniors alone often won’t maintain the data consistently.
