MissSignal is a mobile + web near-miss reporting tool built for frontline speed and manager follow-through. Workers submit a near-miss in under 30 seconds using voice-to-text, quick photo capture, and a simple “what almost happened” prompt—no long forms. The web dashboard turns reports into a lightweight corrective-action workflow: assign an owner, set a due date, and track closure with evidence. The AI layer auto-tags hazards (e.g., slip/trip, pinch point, forklift, chemical), detects duplicates, and drafts a one-paragraph incident summary for safety meetings. It also flags “repeat location / repeat equipment” patterns so managers stop ignoring the same issues. Offline mode supports warehouses and job sites with poor connectivity. The goal is not fancy analytics—it’s reducing the gap between reporting and remediation, which is where most programs fail.
