NeuroFollow

NeuroFollow is a mobile + web app for neurologists and their patients to run structured, between-visit monitoring for common chronic conditions (migraine, Parkinson’s, MS, epilepsy). Patients complete short, validated symptom check-ins, medication adherence logs, and event diaries (e.g., seizures, falls, aura) in under 2 minutes. Clinicians get a web dashboard that summarizes trends, flags clinically meaningful changes, and generates a visit-ready one-page report that can be uploaded into the EHR. This is not a diagnostic tool; it’s a workflow tool to reduce “I don’t remember” visits and to catch deterioration earlier. The MVP focuses on one condition (migraine or epilepsy) to avoid boiling the ocean, with configurable check-in schedules and simple rule-based alerts. The hard part is adoption: it must be faster than current nurse phone calls and portal messages, or it won’t be used.
