NoteGuard

NoteGuard is a web app (with optional EHR-embedded launch) that runs real-time documentation and compliance checks on clinical notes before they’re signed. It flags missing elements tied to payer policies and common audit triggers (e.g., medical necessity language, time-based billing support, MDM completeness, required attestations, diagnosis-plan alignment). Instead of generic “write better notes” advice, it produces a short, actionable checklist per encounter and specialty, with links to the exact policy rule that triggered the flag. It also generates an audit-ready change log and a weekly risk dashboard for compliance leads. This is an AI + rules hybrid: deterministic rules for defensible compliance, plus AI to map note text to required elements and suggest minimal edits. The goal is fewer denials, fewer takebacks, and less back-and-forth between coders and clinicians without turning the workflow into a bureaucratic nightmare.
