NoteWitness is a mobile + web AI-assisted patient advocacy app that helps patients and caregivers document care in a way that holds up when disputes happen. It guides users to record structured visit notes (symptoms, meds, promises made, next steps), captures consent-based audio when allowed, and auto-generates a clean “Care Summary” with dates, participants, and action items. It then builds a timeline of events and produces exportable packets for second opinions, complaints, or insurance/HR escalation. The app includes a simple “Ask for what you need” script library (e.g., prior auth status, discharge planning, medication reconciliation) and reminders to follow up. Reality check: it won’t magically win appeals or fix the system, but it will reduce missing details, conflicting stories, and lost paperwork—the stuff that quietly derails care and coverage.
