OncoHandoff

OncoHandoff is a web + mobile app that standardizes oncology care transitions between clinic, infusion center, hospital, and home health. It replaces scattered phone calls, faxed notes, and “who owns this?” confusion with structured handoff checklists tailored to common workflows (new start chemo, post-infusion fever call, neutropenia admission, discharge to home). Teams can assign owners, set time-bound tasks, and capture critical context: regimen, cycle day, last labs, line type, recent toxicities, dose modifications, and red-flag thresholds. Patients get a simplified view: what to monitor, when to call, and where to go. It integrates with calendars and can ingest key data via HL7/FHIR where available, but still works in low-integration environments using secure forms. This is a traditional app with light AI assistance for summarizing notes and generating patient-friendly instructions (with clinician approval).
