OpNoteForge is a web + mobile AI-assisted documentation app for surgeons that converts rough intraoperative notes (typed, dictated, or templated) into a structured operative report, discharge instructions, and a suggested CPT/ICD-10 coding checklist. The core value is speed and completeness: it prompts for missing critical elements (e.g., laterality, implants, complications, EBL, specimens) and outputs hospital-compliant sections that can be copied into the EHR. It also creates patient-friendly discharge instructions tailored to the procedure and flags potential documentation risks (e.g., ambiguous margins, missing consent language) without pretending to provide medical advice. Realistically, the hardest part is integration and trust—so the MVP focuses on “copy/paste + PDF” outputs and a tight set of common procedures (e.g., lap chole, appendectomy, hernia). The app logs edits to show what the surgeon changed, building confidence over time.
