PostMortemly is a web + mobile app that helps teams and solo builders capture failures while they’re still fresh, then convert them into concrete playbooks. Users log an incident (what happened, impact, timeline, decision points), attach evidence (screenshots, links, metrics), and run a guided “blameless” review template. The AI assistant summarizes root causes, highlights cognitive biases (e.g., sunk cost, confirmation bias), and proposes 3–5 specific prevention actions with owners and due dates. The app then nudges follow-through: checklists, reminders, and lightweight audits to see if the same failure pattern reappears. A searchable “Failure Library” lets you browse past mistakes by tag (launch, hiring, ops, product, sales) and reuse lessons in new projects. This is not therapy; it’s pragmatic operational learning for people who keep repeating the same avoidable errors.
