ProofForm

ProofForm is a web app (with optional mobile-friendly capture links) that helps teams build no-code forms designed for compliance-grade data collection. Instead of plain fields, each question can require “proof” attachments (photo, file, signature, timestamp, GPS, device metadata) and an auto-generated audit trail. It targets workflows where bad data is expensive: safety inspections, incident reports, vendor onboarding, insurance claims intake, and regulated checklists. Users can start from industry templates, enforce validation rules, and export a tamper-evident PDF packet for each submission. It integrates with Slack/Teams for alerts and pushes structured data to Airtable/Google Sheets/webhooks. This is a combination traditional + AI app: AI assists in converting a policy/SOP into a form, suggesting required evidence fields, and flagging missing or suspicious submissions. The focus is not “prettier forms,” but defensible records.
