QuietRounds
QuietRounds is a mobile + web remote monitoring app for home-care agencies and family caregivers managing older adults after discharge or with chronic conditions. It combines lightweight daily check-ins (1–2 minute prompts), passive signals (step count, phone usage, optional wearable data), and caregiver notes into a single risk timeline. The app flags meaningful change—missed meds, reduced mobility, sleep disruption, rising symptom scores—then routes it to the right person (family, nurse, agency coordinator) with a clear “what changed” summary. It’s not a replacement for clinicians; it’s a practical early-warning layer that reduces unnecessary calls while preventing missed deterioration. Setup is designed for low-tech seniors: large text, voice prompts, and a “care partner” onboarding flow. Reporting exports to PDF/CSV for care plans and documentation, but avoids pretending to be an EHR.
