ReferralRail

ReferralRail is a care-coordination web app (with a lightweight mobile companion) that tracks patient referrals end-to-end across clinics, hospitals, home health, and community services. Instead of relying on faxes, phone tag, and “did they schedule?” guesswork, it creates a shared referral timeline: who received it, what’s missing, next steps, and deadlines. The app ingests referral documents (PDF/fax scans) and extracts key fields, then prompts staff to confirm or correct them before sending. Patients get simple SMS updates and self-scheduling links, while coordinators see an exception queue for stalled referrals, missing authorizations, and no-shows. It supports basic FHIR connections where available, but assumes many partners won’t integrate—so it’s built to work even in messy real-world workflows. This is an AI + traditional app: AI helps extract and triage; humans stay in control.
