RootCauseHQ is a web app (with optional mobile companion) that standardizes incident postmortems and turns them into trackable operational improvements. Teams log an incident in 3 minutes, capture timeline, impact, and contributing factors, then generate a structured RCA using guided prompts and evidence links (tickets, logs, docs). The app converts findings into corrective actions with owners, due dates, and verification steps, and it nags until closure. A simple “repeat risk” score highlights which fixes are likely to prevent recurrence versus cosmetic changes. It also builds an internal library of past incidents with searchable patterns (service, vendor, shift, location) so new teams don’t relearn old lessons. This isn’t a full ITSM replacement; it’s a focused layer that makes postmortems unavoidable, consistent, and measurable across operations, manufacturing support, and customer support ops.
