RootCauseKit
RootCauseKit is a web app (with a lightweight mobile companion) for running fast, consistent continuous-improvement cycles after operational issues: defects, delays, safety near-misses, customer escalations, or downtime. It guides teams through a structured workflow: capture the event, quantify impact, run 5-Whys/Fishbone, propose countermeasures, assign owners, and verify effectiveness with follow-up checks. The AI component drafts a first-pass RCA narrative, suggests likely contributing factors based on your past incidents, and generates a clear action plan with measurable acceptance criteria—while keeping humans in control. It integrates with Jira/ServiceNow/Slack so improvements don’t die in a spreadsheet. Realistically, this wins only if it’s dead-simple, fast to deploy, and proves ROI by reducing repeat incidents and time-to-closure. Otherwise it becomes “another process tool” nobody updates.
