RootCauseKit

RootCauseKit is a web app (with a lightweight mobile companion) for running fast, consistent root-cause analysis and tracking whether fixes actually worked. Teams log an incident or defect, then follow guided 5-Whys / Fishbone templates that force evidence: photos, timestamps, process data links, and “what changed” notes. The system converts conclusions into countermeasure tasks with owners, due dates, and measurable success criteria (e.g., defect rate threshold for 2 weeks). A simple AI assistant helps summarize incident narratives, suggest likely causal categories, and draft countermeasure hypotheses—but it can’t “close” anything without attached proof and a verification check. Dashboards focus on repeat issues, time-to-verify, and countermeasure effectiveness, not vanity counts. The goal is to reduce recurring problems and stop the common failure mode of RCA documents that look good but don’t prevent recurrence.
