RosterPulse

RosterPulse is a web app (with optional mobile companion) that helps value-based care teams prioritize outreach using a practical, explainable risk-and-action queue. It ingests claims, ADT feeds, and basic EHR exports, then flags patients likely to have avoidable ED visits, readmissions, or medication nonadherence in the next 30 days. Instead of generic risk scores, it produces an “outreach reason” and a recommended next step (e.g., schedule follow-up, med reconciliation, transportation help), with confidence and supporting data points. The MVP focuses on small-to-mid provider groups and ACOs that lack a full analytics department and are tired of dashboards nobody uses. It includes lightweight care-gap tracking, task assignment, and outcome tracking so teams can prove ROI. It’s an AI-assisted app, but keeps humans in control with transparent explanations and audit logs.
