RxRecon
RxRecon is a web app with an optional mobile companion for general practices that continuously reconciles patient medication lists across GP records, recent discharge summaries, and pharmacy dispensing feeds (where available). It flags high-risk mismatches (duplicate therapies, stopped meds still active, dangerous combinations, renal-dose issues, anticoagulant/NSAID overlaps) and generates a short, auditable “action list” for clinicians and pharmacists. It’s not a full EHR replacement and won’t magically fix messy data; instead it focuses on one measurable outcome: fewer medication-related errors and less time spent manually reconciling lists during appointments. The AI component extracts meds from PDFs and free-text letters, but the workflow is built around human confirmation with clear provenance (“where did this med come from?”). Practices can assign tasks, track resolution, and produce safety reports for governance and insurers.
