SDoHSignal
SDoHSignal is a web app (with optional mobile-friendly views) that helps population health teams identify and act on social drivers of health (housing instability, food insecurity, transportation barriers) that are likely to derail care plans and increase avoidable utilization. It ingests claims, ADT feeds, care management notes, and publicly available neighborhood indices, then produces a prioritized outreach list with clear “why flagged” explanations. The product is not a magic black box: it focuses on transparent risk factors, configurable thresholds, and operational workflows (tasks, outreach attempts, referral tracking). It also tracks whether interventions actually changed outcomes (missed appointments, ED visits, readmissions) so teams can stop funding programs that don’t move the needle. Expect messy data and imperfect signals; the app’s value is making that mess actionable and measurable for real teams under time pressure.
