ShiftScribe is a web + mobile AI app for clinicians that converts shift handoffs and quick voice memos into structured, compliant clinical documentation drafts. It targets the most painful gap between care delivery and charting: the end-of-shift scramble, missing details, and inconsistent handoff quality. Users record a 30–90 second summary (or paste rough notes), select a template (ED, hospitalist, ICU, SNF), and the app produces a draft SOAP/H&P/progress note plus a concise handoff checklist (problems, pending labs, meds, tasks, red flags). It also generates a “billing-ready” problem list with suggested ICD-10/CPT prompts (clearly marked as suggestions). Reality check: it will not replace the EHR; it must integrate lightly (copy/export) and be sold as a time-saver, not a clinical decision-maker. Compliance, audit trails, and PHI handling are non-negotiable.
